Appointments

Please bring the following to your appointment: 

  • Copy of current immunization record 
  • List of all current medications 
  • Any important medical paperwork (ex. discharge paperwork for newborns, ER & hospital visits) 
  • Physical/daycare forms that need to be completed during the visit 
  • Current insurance information 
  • Filled out paperwork for new patients 

Well Visits

To keep your children healthy, it is important to bring them in for regular checkups. Please call to schedule at your earliest convenience so you may choose the date that best fits your schedule. 

The American Academy of Pediatrics recommends regular well visits during the first two years of life (1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 2 years and 2.5 years), followed by yearly well visits after the age of two. Please bring shot records with you to each appointment. 

Sick Visits / Walk-ins

We make every effort to schedule appointments for ill children on the day that you call us. 

We are also available for walk-in sick visits during the following times: 
Monday - Friday: 7-8am and 3-4pm 
Saturday: 8-11am 

If you have a medical emergency during office hours, we will do our best to see your child immediately or refer you to the appropriate facility. 

School / Sports physicals

School and sports physicals should be scheduled as early as possible with your child's pediatrician. Please bring all necessary physical/sports participation forms and shot records to your appointment. There is a $10 charge for forms requested outside of the appointment date. 

A copy of both the Kansas and Missouri pre-participation physical forms can be found below. Please answer all questions prior to presenting the form to the physician at your visit.
